Clear Aligners for General Dentists: Case Selection and Planning

Short answer

Clear aligners move teeth in small planned steps using a series of removable trays, often with bonded attachments and interproximal reduction (IPR). General dentists get the best results by selecting suitable cases, understanding the biomechanics and applying the same diagnostic discipline used for fixed appliances.

How aligners move teeth

Each tray in the series moves the teeth a small planned amount, typically a fraction of a millimetre per stage, and works only while it is worn, usually for 20 to 22 hours a day. Attachments bonded to the teeth give the tray something to push against, and IPR creates space when the plan needs it.

Cases that suit a general dentist

Mild to moderate crowding, spacing, mild relapse after earlier treatment, simple rotations of teeth that are not round in cross section, and alignment ahead of restorative work are the usual starting points.

Cases that are more difficult

Severe crowding, large rotations of round teeth such as canines and premolars, significant extrusion, root movement, skeletal discrepancies, and deep or open bites that need vertical control are harder. Poor compliance defeats any plan.

Plan like a clinician

Do not accept the software's setup unchanged. Check the tooth movements, the sequence, the attachments and the IPR against your diagnosis, and plan for refinements. Biomechanics learned through fixed appliances (force direction, anchorage, centre of resistance) transfer directly to aligner planning. See the biomechanics guide. Retention after aligner treatment is as important as after braces.

Key takeaways

  • Start with mild to moderate crowding or spacing.
  • Attachments and IPR are part of the biomechanics, so plan them deliberately.
  • Fixed appliance knowledge makes aligner planning better.

Frequently asked questions

Can a general dentist offer clear aligners?

Yes, within the rules of the dental council and with adequate training in diagnosis, case selection and aligner biomechanics.

How long does aligner treatment take?

It depends on the complexity of the case and on how consistently the trays are worn. Mild cases take several months and complex cases take longer.
